Hi,

I do not know where you can get the code but if you want to have
Simulink Example for QAM, have a look at Commuication Examples for
ADSL Simulation (Do not exactly remember if it was in Comm or DSP
blockset because it was some time ago when i was working on Matlab)

This simulink Example implements 256 QAM along with other blocks too.
You can get the relevant functions for QAM implementation from the
Simulink blocks.
